By Telephone

In Nevada call: (775) 323-8307

Speak with the person in charge of the department that will be handling the assignment, or with one
of our other knowledgeable staff members. For surveillance investigations ask for Sean Destefans, and for AOE/COE or liability investigations ask for Jim Kleuh. Our staff can direct you to the appropriate person for
other types of assignments. Discussing the details of your case with one of our staff members can be helpful, especially when there is a question about what information may be needed to complete the investigation,
what type of investigation would be most productive for you, or if the investigation is needed on a rush basis.

immediately if there are any questions about your request.

If you do not hear from us within 24 hours, please follow-up with a telephone call to ensure the assignment was received properly.

By Fax
If you feel it would be more efficient to do so, please feel free to fax us your request at (916) 925-4301
using our printable assignment form

which is available from the link below. The assignment form is designed to assist you in sending us all the
basic information that we need to get started on your investigation request. We would also encourage
you to send any supporting documentation that you feel may assist us in conducting the most thorough investigation possible.  If you do not hear from us within 24 hours, please follow-up with a telephone call
to ensure the assignment was received properly.

If you prefer, and if your request is not needed on a rush basis, please also feel free to send us your assignments by mail. This may be the most efficient way to send assignments with a significant amount
of supporting documentation such as claims notes, medical reports, deposition transcripts, etc. We will
